Instructions and Explanatory Notes

Proposals for funds to be awarded to the College must be signed by the Provost. In order to provide adequate time to consider the implications of any College commitments to the project, the proposal materials described in #11 below must reach the Provost's Office at least one week before the proposal must be mailed to meet the application deadline.

1.____ This work does not involve laboratory animals or human subjects.
2.____ This work does not involve recombinant DNA, hazardous chemicals or radioactive materials.
3.____ No computer equipment or external computing services will be acquired.
4.____ New space or renovations will not be required.
5.____ No new equipment will be required that is not provided for in the proposal budget.
6.____ No supplies, maintenance or other costs will be incurred that are not provided for in the budget. If a department budget will provide supplies, maintenance or other costs, the Chair must sign here to indicate this agreement.

7.____ There is no commitment of institutional cost sharing.
8.____ No paid student or other technical assistants will be involved for whom support (including summer student housing) is not provided for in the proposal budget.
9.____ The proposal budget provides the current rates for summer student stipends ($350/week), indirect costs (65% on salaries and wages), and fringe benefits (23% (1st year) for support personnel, 32% for academic year faculty and support staff salaries, and 1.0% for academic year student salaries; 9.0% for student and faculty summer salaries).
10.___ This proposal will not require release time from academic year duties.
11.___ There is no conflict of interest to disclose.
12.___ The original and one copy of the proposal cover sheet, the project summary, the budget, the narrative, and detailed explanations of any items not checked above or other special considerations (with commentary by the department or SEC or DRFC chair where appropriate), are included with this form.
