Personal Health Assessment

FOR A LIMITED TIME ONLY: Fill out your confidential PERSONAL HEALTH ASSESSMENT and receive a $15 gift certificate!

Note: The Personal Health Assessment is currently available for members of HMO Blue, Blue Choice and Blue Care Elect.

First, you will need to register for Member Self Service on the BlueCross/BlueShield site. This will allow you to review your benefits, check the status of a claim, access the online pharmacy, change your primary care physician, request a new ID card, and change your address -- all in one place and all online!

Second, you will set up a MyBlueHealth health information profile. This will allow you to access personalized information on exercise, nutrition, and other health care issues of interest to you, in addition to workshops and programs to help you make healthy choices and healthy changes in your life.

Third, you will complete the Personal Health Assessment. This will give you baseline data about your current lifestyle and health choices (your "HQ" or Health Quotient), show how your HQ compares to that of your peers, and use tools for determining how changes you make (such as diet and exercise) may affect your HQ.

This is completely confidential -- Williams College DOES NOT have access to this information.

You might want to print out these instructions to have on hand as you go through the process.

STEP 1 -- REQUEST A PASSWORD

  • Go to www.bluecrossma.com

  • Click "Member Self Service" in the upper righthand corner.

  • Click "Register Now" near the bottom of the lefthand column.

  • Enter your name, member ID (on your ID card), and date of birth.

  • Click "Submit."

  • You will be given a unique username. Write it down! You will need it every time you log on to to your Member Self Service account.

  • Your temporary password will be mailed to your home address.

STEP 2 -- SET UP YOUR MEMBER SELF SERVICE ACCOUNT

  • Go to www.bluecrossma.com

  • Click "Member Self Service" in the upper righthand corner.

  • In the lefthand column, enter your username and temporary password then click "Go." (see below)

  • The system will prompt you to create a new password.

  • The system will then prompt you to log on again, using your username and new password.

  • Continue to Step 3

STEP 3 -- SET UP YOUR MyBlueHealth PROFILE

  • Your account profile will appear including your name, home address and Plan information. In the lefthand column are links for managing your account. Click "Learn More" under the MyBlueHealth hand holding the apple (see below).

  • The next page explains the features and tools available on the MyBlueHealth site. Click "Go Now" under the MyBlueHealth hand holding the apple.

  • A message will appear letting you know that you are leaving the Member Service Center and going to MyBlueHealth (sponsored by WebMD). Click "Continue."

  • Follow the prompts to set up your health information profile. Read and check the Privacy Policy. Then click "submit."

  • Select topics of interest to you, then click "Continue."

  • Your MyBlueHealth website will be created. This will allow you access to news articles and links related to the options you chose in filling out the health information profile.

  • Continue to Step 4.

STEP 4 -- FILL OUT YOUR PERSONAL HEALTH ASSESSMENT

  • Click "Personal Health Assessment" (see below).

  • In the lefthand column under "My Assessment," click "Take Assessment."

  • A Data Share Agreement will pop up. You must elect to share your information to qualify for the $15 gift certificate. (Any information shared from your assessment is used without your name.)

  • Follow the instructions to complete the assesssment.

  • You will receive your results including your "HQ" (health quotient) and how you compare to your peers. The four sections of your report provide not only your test results but also suggestions to improve your health and to stay healthy. Take the time to read yours carefully. The pages can be printed for future reference and/or to share with your doctor (click on the printer icon on the right side of the pages you wish to print).

  • Your assessment will stay on your MyBlueHealth site where you can review and update it as you get new test results from your doctor or make changes in your lifestyle.

  • You might be asked to complete a short survey about the assessment itself and about your experience in using it.

  • Continue to Step 5

STEP 5 -- RECEIVE YOUR $15 GIFT CERTIFICATE

  • When you have finished reviewing your health assessment, you will be returned to the My Health Guide page. Scroll down until you see Wellness Rewards. Click "Click here to view your rewards status."

  • Under Redeem Your Rewards, click "Redeem Now" to the right of "Reward for completing Personal Health Assessment."

  • You will be prompted to select whether to receive your reward via e-mail or at mailed to your home address.

  • Close the Rewards window. This will return you to the My Health Guide page.

  • Scroll to the top of the page and click "Log Out." Close the Thank You window. This will return you to the Member Self Service page.

  • Click "Log Out" on the Member Self Service page.

To return to your Personal Health Assessment:

If you want to review your report, add information, or note changes you have made in your lifestyle, you can return to your Personal Health Assessment at any time. In addition, you may re-take the assessment every six months. A record of your progress will be kept on your MyBlueHealth site.

  • Go www.bluecrossma.com

  • Click "Member Self Service" in the upper right of the page.

  • Enter your username and password. Click "Go."

  • Click "Learn More" under the hand holding the apple.

  • Click "Go Now" under the hand holding the apple.

  • Click "continue" in the pop-up box.

  • Select "Personal Health Assessment."

  • Select "View Report."

  • As long as you've gone to the trouble of getting here, take the time to explore the many other offerings on these pages as well!

  • Don't forget to log out of both MyBlueHealth and Member Self Service when you're done.
