Students' medical records are confidential. To obtain a copy of medical records, the student needs to complete a medical record request form obtained from the Health Center or provide us with a request in writing. The request should include:

Full name

Class year

Date of birth

What records are needed

Where the records should be sent/faxed

Student signature
